Introduction: Iatrogenic Alogenosis has become an increasingly common condition affecting individuals seeking aesthetic enhancement and physical perfection. This growing phenomenon carries significant health risks and complications. In this report, we present a case involving autologous fat injection in the genital area. Moreover, patients by this condition often require comprehensive, multidisciplinary care. Objective: The objective of this report is to present a clinical case of a male patient diagnosed with iatrogenic alogenosis in the genital area, along with its management and treatment. Materials and Methods: This is the case of a 36 year old man with no history of substance use, the patient underwent autologous fat transfer to the penis for aesthetic reasons, he developed localized swelling that progressed to purulent penile necrosis, clinical evaluation, surgical drainage, and multidisciplinary care were required. Imaging and case review confirmed a diagnosis of iatrogenic alogenosis. Conclusion: Genital enhancement procedures can lead to severe complications even with autologous materials, these include tissue necrosis, infection, and systemic symptoms, multidisciplinary care should involve mental health support, early detection is essential to prevent physical and psychological consequences, psychological care reduces recurrence and improves treatment outcomes.
