Eficacia clínica del abordaje quirúrgico frente al conservador en pacientes con quemaduras de segundo grado: revisión sistemática

Resumen

Introducción: Las quemaduras de segundo grado representan un desafío por su alta incidencia y variabilidad pronóstica, comprendiendo aproximadamente entre el 60 y el 70% de todas las quemaduras que requieren atención especializada. La principal controversia en el manejo de estas lesiones se centra en las quemaduras dérmicas profundas (SDp), donde la dificultad para predecir la reepitelización espontánea frente a la necesidad de cirugía condiciona directamente la toma de decisiones terapéuticas. Esta revisión sistemática tuvo como objetivo determinar la eficacia clínica comparativa del abordaje quirúrgico frente al manejo conservador en pacientes con quemaduras de segundo grado. Materiales y métodos: Se desarrolló bajo la metodología PRISMA 2020, realizando una búsqueda sistemática en las bases de datos PubMed/MEDLINE, Scopus, Embase y Web of Science entre abril y mayo de 2026, identificando 4,218 registros iniciales. Tras la eliminación de duplicados y la aplicación de criterios de elegibilidad, se seleccionaron 13 estudios publicados entre 2021 y 2026 para su inclusión definitiva. Resultados: La síntesis de la evidencia analizada revela que la escisión temprana convencional reduce el tiempo de cicatrización (17.6 vs. 40.2 días) y la estancia hospitalaria; el desbridamiento enzimático con NexoBrid® logró una reducción de la escisión quirúrgica del 48.1% al 1.5% y una tasa de cicatrización sin injerto del 39%; la hidrocirugía demostró mejor preservación dérmica (p < 0.001) y puntuaciones POSAS significativamente mejores (2.42 vs. 2.54; p = 0.023). Discusión: Los hallazgos confirman que no existe una estrategia terapéutica única; los resultados avalan una jerarquía terapéutica en la que la elección de la técnica debe personalizarse según la extensión, profundidad, zona anatómica y objetivos funcionales del paciente. La escisión temprana mantiene su indicación como estándar para el control de riesgos sistémicos, mientras que tecnologías avanzadas como el desbridamiento enzimático y la hidrocirugía ofrecen resultados superiores en preservación dérmica y calidad cicatricial. Conclusiones: Se concluye que el manejo de las quemaduras de segundo grado debe orientarse a preservar tejidos, reducir complicaciones y mejorar la calidad de vida a largo plazo, con una selección adecuada de la técnica como factor determinante para optimizar los resultados funcionales y estéticos.
