Eficácia clínica do tratamento cirúrgico versus conservador em pacientes com queimaduras de segundo grau: uma revisão sistemática

Resumo

Introdução: As queimaduras de segundo grau representam um desafio devido à sua alta incidência e prognóstico variável, compreendendo aproximadamente 60-70% de todas as queimaduras que requerem cuidados especializados. A principal controvérsia no manejo dessas lesões centra-se nas queimaduras dérmicas profundas (QDP), onde a dificuldade em prever a reepitelização espontânea versus a necessidade de cirurgia influencia diretamente as decisões terapêuticas. Esta revisão sistemática teve como objetivo determinar a eficácia clínica comparativa do tratamento cirúrgico versus o tratamento conservador em pacientes com queimaduras de segundo grau. Materiais e métodos: A revisão foi conduzida utilizando a metodologia PRISMA 2020, realizando uma busca sistemática nas bases de dados PubMed/MEDLINE, Scopus, Embase e Web of Science entre abril e maio de 2026, identificando 4.218 registros iniciais. Após a remoção de duplicatas e a aplicação dos critérios de elegibilidade, 13 estudos publicados entre 2021 e 2026 foram selecionados para inclusão final. Resultados: A síntese das evidências analisadas revela que a excisão precoce convencional reduz o tempo de cicatrização (17,6 vs. 40,2 dias) e o tempo de internação hospitalar; O desbridamento enzimático com NexoBrid® resultou em uma redução na excisão cirúrgica de 48,1% para 1,5% e uma taxa de cicatrização sem enxerto de 39%; a hidrocirurgia demonstrou melhor preservação dérmica (p < 0,001) e escores POSAS significativamente melhores (2,42 vs. 2,54; p = 0,023). Discussão: Os achados confirmam que não existe uma estratégia terapêutica única; os resultados apoiam uma hierarquia terapêutica na qual a escolha da técnica deve ser personalizada de acordo com a extensão, profundidade, área anatômica e objetivos funcionais do paciente. A excisão precoce permanece o padrão de tratamento para o manejo dos riscos sistêmicos, enquanto tecnologias avançadas como o desbridamento enzimático e a hidrocirurgia oferecem resultados superiores na preservação da pele e na qualidade da cicatriz. Conclusões: O manejo de queimaduras de segundo grau deve focar na preservação do tecido, na redução de complicações e na melhoria da qualidade de vida a longo prazo, sendo a seleção da técnica apropriada um fator determinante para a otimização dos resultados funcionais e estéticos.
