Abstract

Introduction: Laparoscopic cholecystectomy has been considered the standard treatment for symptomatic cholelithiasis due to its favorable clinical outcomes and lower morbidity compared with open surgery. Adequate cystic duct closure represents a critical step in preventing postoperative complications. Although titanium clips have traditionally been the most widely used closure method, advanced energy devices such as LigaSure® have emerged as potential alternatives. Objective: To analyze postoperative outcomes associated with cystic duct sealing using titanium clips and LigaSure® in patients undergoing elective laparoscopic cholecystectomy. Materials and Methods: An observational, analytical, retrospective, and cross-sectional study was conducted through the review of 222 medical records of patients who underwent elective laparoscopic cholecystectomy. Participants were equally distributed according to the sealing method used: titanium clips (n = 111) and LigaSure® (n = 111). The association between variables was assessed using Pearson’s Chi-square test, considering a significance level of p < 0.05. Results: The overall incidence of postoperative complications was 1.8%. Four complications were identified in the LigaSure® group (3.6%), whereas no postoperative complications were documented among patients treated with titanium clips. All recorded complications corresponded to bile leakage. Statistical analysis demonstrated a significant association between the sealing method and the occurrence of postoperative complications (χ² = 4.073; p = 0.044). Conclusions: Postoperative complications were uncommon in the studied population. Although a significant association was observed between the use of LigaSure® and the occurrence of bile leakage, the low frequency of events requires cautious interpretation and limits the ability to establish definitive conclusions regarding the superiority of one sealing technique over the other.
